Someone Cares Inc. is a 501(c)(3) nonprofit Certified Community Behavioral Health Clinic serving the Atlanta metro area for over 30 years. It provides primary care, behavioral health, HIV testing and prevention, and addiction treatment including drug screening, a specialized opioid treatment program and a partial hospitalization program, with mobile health units extending care into the community. The organization accepts Medicare and Medicaid.

Outpatient Rehab

Community Partners Integrated Healthcare was one of the first licensed integrated care clinics in Arizona, and has grown from a single clinic serving 40 members in 2014 to multiple locations across eight counties. Integrated means what it says here: mental health, substance use and physical healthcare are delivered by one organization rather than coordinated between three. It was a catalyst for the best-practice crisis system now used in Southern Arizona and shares that expertise with other providers.

Advanced Therapeutic Services runs outpatient addiction and behavioral health treatment from two Southern California sites, in Barstow and Rancho Mirage. Its clinical lineage is unusual for an outpatient program: co-founder Jim Herndon co-authored an abstract in the ASAM Journal of Addictive Diseases in 2005 finding that buprenorphine detoxification improved patient retention, and that protocol went on to be recognized nationally as a model for opiate detox. Josie Ramirez-Herndon, the founder and chief executive, has worked in behavioral health and addiction services since 1987. The program is a Blue Distinction Center by Anthem Blue Cross and holds CARF accreditation.

Part of the EMCMHC network serving eastern Montana since 1967, the facility provides chemical dependency assessments, individual and group therapy, medication management, and peer support services. Medical staff are on site 24 hours and work to SAMHSA evidence-based protocols, and the program is accredited by The Joint Commission. Its medication-assisted treatment covers all FDA-approved medications except methadone, which is worth knowing before applying if methadone is what you are stabilized on. Michigan Medicaid is accepted from specific counties and regions, with navigators who help clients apply; people who do not qualify can self-pay or be assessed for income-based payment. Medicare, private and military insurance and state funding are also accepted.

Pacific Sands Recovery Center is a boutique detox and residential addiction treatment facility in Santa Ana, Orange County, offering just six beds — four in private rooms — for individualized care. The program treats drug and alcohol addiction, dual diagnosis, and complex medical conditions, with services including medically supervised detox, residential treatment, medication-assisted treatment, and 12-step programming. Licensed by the California Department of Health Care Services and JCAHO accredited, Pacific Sands also offers a dedicated rehab track for veterans. The facility accepts major insurance plans including Anthem Blue Cross, Aetna, TRICARE, TriWest, GEHA, Magellan, and Carelon.

North Florida Evaluation and Treatment Center is a state-operated forensic residential facility in Gainesville, run by the Florida Department of Children and Families. It provides court-ordered evaluation and residential treatment for adults in the criminal justice system who have mental health and substance use needs. Admission is by referral through Florida's judicial system rather than by self-referral or family application, so the usual questions of insurance and cost do not apply here — but neither can a place be requested directly.

As a U.S. Department of Veterans Affairs facility, it offers individual and group counselling, relapse prevention, and aftercare planning. Fernwood Counseling Center is the Mount Rogers Community Services Board's site for Carroll and Grayson counties and the city of Galax, offering counseling, psychiatry and intensive outpatient care for mental health and substance use disorders, with same-day access intakes every weekday — so a person who decides to seek help on a Tuesday morning does not wait weeks for an assessment. Mount Rogers is a not-for-profit governed by a fourteen-member board and has served southwest Virginia since 1972. Uninsured or underinsured clients may be given a capped fee under its sliding scale, or can apply for financial assistance; Medicaid, Medicare, private and military insurance and state funding are all accepted.

It sits within a network running residential, partial hospitalization and intensive outpatient programs across Illinois and Wisconsin, including adolescent residential care, and an alumni program after discharge. Rosecrance is a Qualified Veterans Community Care Network provider, and its Kinley Fund provides charity care to families who cannot pay. Locations are certified for Medicaid and government-sponsored insurance, and Medicaid, Medicare, private and military insurance are accepted, including Aetna, Blue Cross Blue Shield, Cigna, Humana, Optum and TRICARE.

Private and military insurance and self-pay are accepted.

Life Center of Hillsville is a residential addiction treatment program for men age 18 and older, located on a 100-acre campus in Hillsville, Virginia. Part of the Life Center of Galax network, it offers a clinically managed 24-hour residential program and a partial hospitalization program (PHP), supported by a multidisciplinary team of doctors, nurses, and counselors. Life Center of Hillsville accepts Virginia Medicaid and most commercial insurance, but does not currently accept Medicare.
