Turning Point House by Caz Recovery is a residential rehabilitation program for men with substance use disorders, certified by the New York State Office of Addiction Services and Supports. Located on four acres of wooded rural property in Eden, the program serves 30 men age 18 and older and offers substance use, mental health, vocational, and family counseling alongside basic medical care. Admission requires enrollment in a Medicaid managed care plan.

Bee Cave Recovery is a community-based nonprofit treatment center near Austin, Texas offering intensive outpatient treatment for drug and alcohol addiction. Its program uses a 12-step model, focusing on the first three steps and relapse prevention, and provides aftercare groups, meditation, and individual and family counseling through a hybrid of virtual and in-person sessions. It accepts Medicare.

Outpatient Rehab·Mental Health Center

NuLife Behavioral Health's Worcester program offers three levels of outpatient care: a partial hospitalisation program with about five hours of therapy, groups, skill-building and medication management per day, an intensive outpatient program for people who need more than weekly therapy or who are stepping down from PHP, and a general outpatient program of weekly individual or group therapy. The provider also offers psychiatric medication management, TMS therapy and dual-diagnosis treatment, and describes a single phone call leading to a same-day or next-day assessment, insurance verification and placement. Medicaid and Medicare are accepted, and the carrier list includes Aetna, Blue Cross Blue Shield of Massachusetts, Cigna, Humana, Magellan, Meritain, Optum and TRICARE.

Treatment Center

The Extension is a licensed, nonprofit long-term residential recovery program in Marietta, Georgia for homeless men and women of Cobb County living with alcohol and other addictions. It operates a 78-bed campus for men and a 20-bed campus for women, with more than 25 years of service, combining group and individual counseling with education, life-skills training and job placement within weeks of arrival.

Unhooked Recovery runs detox and residential treatment in Arizona, and its intake is a face-to-face biopsychosocial assessment scored against the ASAM dimensions — the standard framework for deciding which level of care a person actually needs. Naming the framework matters: it is the difference between a placement decision a clinician can defend and one made by whichever bed is free.

Van Buren Community Mental Health (VBCMH) is a Certified Community Behavioral Health Clinic (CCBHC) in Paw Paw, Michigan, serving Van Buren County residents of all ages with outpatient, home-based, and school-based behavioral health and substance use services, plus a 24/7 crisis line. Care covers serious mental illness, intellectual and developmental disabilities, serious emotional disturbance, and substance use disorders, with a dedicated Veteran Navigator program. VBCMH accepts Medicaid and Medicare and offers a sliding fee scale based on family size and income, with no one denied services for inability to pay.

Sun Ray Court is the men's residential program of New Horizon Care Centers (NHCC) in Spokane, Washington, treating substance use disorders alongside mental health and gambling disorders. NHCC also operates the women's Isabella House and outpatient services, with a sliding fee scale for private pay and acceptance of Medicaid. A National Health Service Corps site, New Horizon Care Centers is a nonprofit organization.

Four Corners Community Behavioral Health is a community behavioral health provider serving southeastern Utah, with a clinic in Moab. It offers intensive outpatient and outpatient treatment for substance use and mental health conditions on a scheduled basis. Care is available to residents of the surrounding rural communities.

The Tallahassee Outpatient Annex is part of the North Florida/South Georgia Veterans Health System, providing outpatient substance use and mental health treatment to veterans. As a U.S. Department of Veterans Affairs facility, it offers individual and group counselling, relapse prevention, and aftercare planning. The facility accepts Medicaid, Medicare and private insurance.

It is nationally dual-accredited by CARF for intensive outpatient and outpatient treatment of substance use disorders, and offers comprehensive assessments, cognitive behavioral therapy, co-occurring-capable care and aftercare for adults, and accepts Medicare and Medicaid.

Bella Nirvana Center provides medically monitored detox, residential treatment, intensive outpatient care and sober living in Folsom, in the Sacramento area. Detox is run as distinct alcohol, opioid and cocaine programs, staffed around the clock by detox nurses and health technicians alongside CADC counselors, with a doctor and psychiatrist for clients with a dual diagnosis. An outpatient medication-assisted treatment program follows, aimed at preventing relapse. The center is licensed by the California Department of Health Care Services (license 340007AP), and veterans can be covered through their VA community care benefits. The program takes private and military insurance, and clients can also pay privately.

North Florida Evaluation and Treatment Center is a state-operated forensic residential facility in Gainesville, run by the Florida Department of Children and Families. It provides court-ordered evaluation and residential treatment for adults in the criminal justice system who have mental health and substance use needs. Admission is by referral through Florida's judicial system rather than by self-referral or family application, so the usual questions of insurance and cost do not apply here — but neither can a place be requested directly.

The Work Group Counseling Center, also known as Joe's Place, provides outpatient individual counseling in Pennsauken, New Jersey, for adults age 16 and up with substance use and co-occurring mental health concerns. The program offers medication evaluation and monitoring, family and couples counseling, and aftercare services, and currently accepts Medicaid.
