Acqua Recovery is a residential addiction and mental health treatment center on Johnson Mill Road in Midway, Utah, in the Wasatch Back away from the city. Community Mental Health Affiliates (CMHA) is a private non-profit behavioral health provider headquartered in New Britain, Connecticut, with eight locations across central Connecticut and in Waterbury. It was the state's first fully Joint Commission accredited Behavioral Health Home and is a SAMHSA-certified Community Behavioral Health Clinic. Its residential continuum includes a 12-bed supervised program staffed by an Intensive Assertive Community Treatment team; the 16-bed Parkview 5, which CMHA describes as one of only two programs at that level of care in the state, with an average stay of about 18 months and usually a waiting list; and a 19-bed Transitional Living Center that steps clients down from Parkview 5 with nursing cover and two respite beds. Medicare, private insurance and self-pay are accepted.

McAlister Institute provides residential substance use treatment in Lemon Grove, California. Founded in 1977, McAlister Institute has grown into one of San Diego County’s largest alcohol and drug treatment providers, operating 24 programs across a full continuum of care and serving more than 13,000 individuals annually. McAlister Institute is a 501(c)(3) nonprofit organization offering prevention, education, outreach, outpatient, short-term and long-term residential care, aftercare, and sober living. The organization serves men, women, teens, and children and accepts military insurance and state-financed health plans.

Nirvana Recovery provides residential addiction treatment in Phoenix with medically supervised detox support at the front of it, then intensive programming, and — named separately — aftercare planning and intervention services. The intervention offer is the unusual one: it is help for the family trying to get someone into treatment, which is a different service from treating the person once they arrive.

North Florida Evaluation and Treatment Center is a state-operated forensic residential facility in Gainesville, run by the Florida Department of Children and Families. It provides court-ordered evaluation and residential treatment for adults in the criminal justice system who have mental health and substance use needs. Admission is by referral through Florida's judicial system rather than by self-referral or family application, so the usual questions of insurance and cost do not apply here — but neither can a place be requested directly.
