Samaritan Medical Center is a 290-bed not-for-profit community hospital in Watertown, New York, serving the North Country's civilian and military communities since 1881. Its addiction services are delivered as an intensive outpatient program, alongside the hospital's wider inpatient and outpatient care. A financial assistance program is available to patients who are uninsured, underinsured or facing financial hardship. Medicaid, Medicare and private or military insurance are accepted, and state funding is available.

Metropolitan Family Services provides behavioral health and substance use treatment at its community center at 11560 South Kedzie Avenue in Chicago. Green River Medical Center, operating as Two Rivers Community Health, is a Federally Qualified Health Center in Green River, Utah. The clinic provides primary and preventive medical care alongside behavioral health services, with detoxification services available. As an FQHC, it offers care on a sliding fee scale and accepts Medicare, Medicaid, and private insurance.

Crescent Moon Recovery is a residential drug and alcohol treatment center in Fountain Valley, California, offering gender-specific programs for men and women alongside detox, partial hospitalization, intensive outpatient, and outpatient care. Amethyst House provides residential treatment in Staten Island, New York. The facility accepts Medicare and private insurance.

The Commitment House Men's residential treatment facility is located on 725 New Glendale Rd, Elizabethtown. The facility accepts clients on opioid medication.

Twin Town Treatment Centers provides intensive outpatient treatment for adults and adolescents with substance use disorders in Laguna Hills, California. Accredited by The Joint Commission for over 25 years, the program offers day and evening IOP tracks. Twin Town accepts most major insurance plans including network-model commercial coverage in California, Medicare, and military insurance.

Odyssey House Louisiana provides residential substance abuse treatment in Lake Charles, Louisiana. The program is part of the Odyssey House LA network based in New Orleans and offers structured residential programming with a focus on long-term recovery. The Life Center of Connecticut, which trades as Youth Challenge, began in 1970 as a residential outreach center for men and women in the Greater Hartford area and now runs a men's residential center, a men's transitional program and outpatient services from May Street in Hartford, alongside Esther's Home, a sober-living house for fifteen women. Treatment integrates faith-based support with evidence-based clinical care, delivered by licensed clinicians, and the programs emphasize employment readiness, family support and community connection. Participants at Esther's Home are expected to take part in its faith-based activities, which is worth knowing before applying. Medicaid, Medicare, military insurance, state funding and self-pay are accepted.

Absolute Control Treatment Center in Burbank, California offers a full continuum of care spanning residential treatment, medically supervised detoxification, outpatient programming, and sober living. The program treats substance use disorders alongside co-occurring mental health conditions. Absolute Control accepts private insurance, military insurance, and state-financed health plans.

Center for Discovery's Addison site opened in May 2016 and treats eating disorders and co-occurring conditions with treatment plans customized per patient rather than run to a fixed program. Clinicians across the organization are trained in EMDR, trust-based relational intervention and motivational interviewing, and the site sits within a national network, so a patient who needs a different level of care or a different location can usually stay inside it.

Harmony is a state-of-the-art residential addiction treatment program, confidentially nestled on a 43 acre campus in the Rocky Mountains, just outside of Estes Park, Colorado.

TCV Community Services, part of Devereux Advanced Behavioral Health, provides residential substance use treatment in Homestead, Pennsylvania. The programme serves adults and offers individualized care within Devereux's Advanced Philosophy of Care framework, which emphasizes positive behavioral interventions and trauma-informed practices. TCV accepts Medicaid, Medicare, private insurance, and state-financed coverage.

Evolve Adolescent Behavioral Health has provided teen and adolescent treatment for mental health and addiction since 2014. Its San Diego-area Dove Song location offers residential treatment, including a newly opened neuroaffirming program designed specifically for autistic teens. The Recovery Center at St. Luke's Penn Foundation in Sellersville provides a full continuum for substance use: an inpatient, non-hospital detox giving 24/7 support outside a traditional hospital setting, inpatient rehabilitation that treats substance use and co-occurring mental health conditions together, and intensive outpatient and outpatient care. Treatment begins with an assessment and can draw on psychological and psychiatric evaluation, medication management, and individual, family, marital, group and faith-based counseling. A peer-support service is open to adults in Upper Bucks and Upper Montgomery counties who have Medical Assistance or may qualify for county funding. Medicaid, Medicare, private and military insurance are accepted, along with state funding, self-pay and a sliding scale.
