Affinity House is a residential program for women in Eau Claire, operated by Lutheran Social Services of Wisconsin and Upper Michigan — a human-services organization whose work in the state traces back to 1882. It offers structured sober housing and recovery support for women, and accepts residents who are receiving medication for opioid use disorder. Medicaid, Medicare, military insurance and public assistance are accepted.

The Lower Eastside Service Center (LESC) provides residential and outpatient addiction treatment in New York City, serving New Yorkers since 1959. Programs include Su Casa residential treatment, outpatient opioid recovery with medication-assisted treatment, and supportive housing in Manhattan and the Bronx. It accepts Medicaid and Medicare.

Gateway Rehab (Gateway Rehabilitation Center) has provided addiction treatment in western Pennsylvania since 1972, with a campus at 1820 Washington Road in Washington, PA offering inpatient rehabilitation, withdrawal management, outpatient programs and medication-assisted recovery. The nonprofit provides 24-hour assessment and admission, and accepts most major insurance including Aetna, Cigna, UnitedHealthcare, Humana, Highmark and Magellan.

Evolve Adolescent Behavioral Health provides residential treatment, partial hospitalization (PHP), and intensive outpatient (IOP) programs for teenagers ages 12 to 17 in California. The program uses evidence-based approaches including dialectical behavior therapy (DBT) and cognitive behavioral therapy (CBT), combined with individual and family therapy, psychiatry, and academic support. Evolve accepts most private insurance plans including Aetna, Anthem, BlueCross BlueShield, Cigna, and United Healthcare. The location offers medically supervised withdrawal management, intensive outpatient and standard outpatient programming, and a recovery home. It accepts most major insurance plans, including Medicare, private insurance, military insurance and state-funded coverage. The operator was known as Gateway Foundation until August 2026; the name changed, this location and its programs did not.
