Amery Hospital & Clinic – Behavioral Health Center provides inpatient and outpatient mental health and chemical dependency treatment in Amery, Wisconsin. Located on the hospital’s west campus, it operates the only inpatient behavioral health program in the northern St. Croix Valley, with care ranging from outpatient therapy to around-the-clock inpatient stabilization. The center is part of Amery Hospital & Clinic, a HealthPartners hospital serving the region since 1956, and accepts Medicare, Medicaid, TRICARE and private insurance.

Fernwood Counseling Center is the Carroll, Grayson, and Galax counseling location of the Mount Rogers Community Services Board, offering counseling, psychiatry, and intensive outpatient care for mental health and substance use disorders. The center provides same-day access intakes every weekday. It accepts Medicaid, Medicare, private insurance, and military insurance.

Sanctuary Sober Living operates structured residential recovery housing in Phoenix, Arizona, for adults. Support addresses substance use and co-occurring conditions, with CBT, DBT, and trauma-related counseling among the approaches offered. Medicaid, Medicare, and private insurance are accepted.

North Florida Evaluation and Treatment Center is a state-operated forensic residential treatment facility in Gainesville, Florida, run by the Florida Department of Children and Families. The facility provides court-ordered evaluation and residential treatment for adults involved in the criminal justice system who have mental health and substance use needs. As a state-operated program, it serves individuals referred through Florida's judicial system.

Muir Wood provides residential and intensive outpatient treatment for adolescents ages 12–17 with primary mental health conditions and co-occurring substance use, at its Riverside, California campus. The program features WASC-accredited academics coordinated with each teen's home school, and an integrated team of psychiatrists, therapists, nurses and educators. Muir Wood is in-network with most commercial insurers and is not in-network with Medi-Cal/Medicaid.

County-operated substance use clinic at 1297 W. Hobsonway in Blythe, part of Riverside University Health System – Behavioral Health. It provides outpatient and intensive outpatient treatment for adolescents and adults, with access through the county's 24/7 CARES referral line.

Metrocare Services is the largest provider of mental health, developmental disability, and substance use services in North Texas, serving Dallas County for over 50 years. In addition to behavioral health care, Metrocare operates accessible pharmacies, provides veterans' services, housing support, and community-based social services. Substance use programs follow a person-centered, trauma-informed recovery model.

NorthStar Regional treats substance use and mental health conditions in the same place and by the same team at its Chaska, Minnesota site, with a high-intensity residential co-occurring programme that runs up to six weeks and a medically managed withdrawal management service at ASAM level 3.7. Step-downs include co-occurring intensive outpatient care with lodging, IOP groups three to five sessions a week, outpatient therapy in person or by telehealth for adults, teens and children, and men's and women's sober housing for clients further along in recovery. The provider states that insurance and public funding are accepted, and recorded options include Medicare, private insurance and state-financed coverage.

Syntero is a 501(c)(3) non-profit that has provided mental health and substance use disorder services in Central Ohio for more than 50 years, working from 299 Cramer Creek Court in Dublin and other sites across Franklin, Delaware and Morrow counties. The organisation is accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF) and certified by the Ohio Department of Behavioral Health, with funding support from the ADAMH Board of Franklin County and the Delaware-Morrow Mental Health and Recovery Services Board; services include intensive outpatient and outpatient treatment, counselling for youth, families and older adults, in-home support, and same-day appointments for young people aged 3 to 18 and school staff in Delaware and Morrow counties. Medicaid and Medicare are both recorded as accepted, alongside private and military insurance and self-pay.

The White River Junction VA Medical Center provides health care to veterans across Vermont and parts of New Hampshire, including mental health care and treatment for substance use disorders. Programming at this site covers residential and outpatient care for veterans with addiction and co-occurring conditions. Care is provided by the Department of Veterans Affairs to enrolled veterans.

Greater Nashua Mental Health is a nonprofit community mental health center on Amherst Street in Nashua that has served the area since it was incorporated in 1924 as the Community Council of Nashua and became a federally recognized Community Mental Health Center in 1974. It provides outpatient and intensive outpatient care for children, adolescents, adults and older adults, specializing in significant behavioral health challenges including co-occurring substance use disorders, and runs homeless outreach and services for people who are Deaf or hard of hearing in New Hampshire. Medicaid, Medicare and most private insurance are accepted, a sliding fee scale is available based on family size and income, and the center states no one is ever turned away due to inability to pay.

Northcutt Place in Dayton is the residential treatment centre of Eastway Behavioral Healthcare, a not-for-profit founded in 1949 and incorporated in 1957 that calls itself the largest private not-for-profit provider of mental health, substance use, housing, employment and educational services in the Miami Valley. The Northcutt Residential Treatment Center provides a residential setting for elementary school-aged boys who have experienced trauma, and sits inside a wider Eastway continuum that includes adult and adolescent outpatient and psychiatric care, forensic services, The Ranch of Opportunity for teenage girls and The Heritage of Hannah Neil on 22 acres in Columbus. Recorded payment options include Medicare, private insurance, state-financed coverage and self-pay.

Womans Christian Assoc of Jamestown in Jamestown is a provider of outpatient mental health services.

Project Hope for Women & Children is an intensive residential treatment program in Huntington for women with substance use disorders and their children, run by Marshall Health and the Joan C. Edwards School of Medicine. The program provides onsite peer and residential support, life-skills training and mental health services so mothers can work toward long-term recovery while developing parenting skills, and the average length of stay is up to six months. Referrals are taken by phone at 304.696.HOPE.

Valley Star in San Bernardino, California provides residential substance use treatment, operated by Stars Behavioral Health Group. The program accepts Medicare and state-funded insurance.

New Day Recovery provides medical detox, residential treatment and an intensive outpatient program for drug and alcohol addiction on Greenwood Road in Shreveport, with a 24/7 supportive residential community and onsite assessments producing psychosocial reports that address both addictive behaviour and co-occurring conditions. Additional support includes the Rally Point Recovery veterans program, and the site states its staff are fully licensed, certified and credentialed. Louisiana Medicaid covers intensive outpatient services here, and Medicare and private insurance are also accepted.

The Comprehensive Behavioral Health Center of Saint Clair County provides withdrawal management, residential, intensive outpatient and outpatient care for substance use disorders in East Saint Louis, Illinois. The non-profit center serves adults and young adults, including veterans and people with co-occurring mental health conditions, and offers methadone and buprenorphine for opioid use disorder alongside cognitive behavioral therapy and motivational interviewing. Medicaid and Medicare are accepted.

Chestnut Health Systems' Granite City site at 50 Northgate Industrial Drive combines primary care with mental health and substance use treatment under one roof, with published weekday hours and walk-in assessment available. The organisation works across Illinois and Missouri, reported 1,229 medication-assisted treatment and recovery clients in 2024, and runs school-based services at more than 80 Illinois locations; its Crisis Residential Unit in nearby Maryville provides 24-hour short-term supervised care for adults 18 and over on a voluntary basis, with an average length of stay of 14 days. Medicare, private and military insurance and self-payment are accepted, and Chestnut states that it offers sliding fee discounts to keep care accessible.

NFI North is a nonprofit behavioral health organization serving children, adults, and families across Maine and New Hampshire, with a residential program in Stetson, Maine, for people managing mental health and co-occurring substance use conditions. Its residential services take a family-driven, trauma-informed approach, with teams of therapists, teachers, and other professionals supporting each person. Accredited by The Joint Commission, NFI North accepts MaineCare (Medicaid), Medicare, and private insurance.
