Bestself Behavioral Health provides outpatient behavioral health and substance use treatment in Dunkirk, New York, including an intensive outpatient program for co-occurring mental health and substance use disorders. Formed in 2017 through the merger of Child & Adolescent Treatment Services and Lake Shore Behavioral Health, it is the largest community-based behavioral health provider serving children and adults across Western New York. Bestself accepts Medicaid and Medicare, offers a sliding fee scale, and does not turn anyone away for inability to pay.

Red Rock Behavioral Health Services is a 501(c)(3) nonprofit community mental health organization founded in 1974 that serves Oklahoma City and 16 counties across Oklahoma, and its Elk City site on West 3rd Street provides outpatient and intensive outpatient care for adults aged 16 and older as well as children's and adolescent services. Red Rock opened an Urgent Recovery Center in Elk City in December 2023, holds CARF accreditation first awarded in 1992, and reports a workforce of nearly 800 staff as of 2025. Thrive Behavioral Health is the Certified Community Behavioral Health Clinic (CCBHC) for Kent County, Rhode Island, delivering mental health and substance use services since 1976. The program features WASC-accredited academics coordinated with each teen's home school, and an integrated team of psychiatrists, therapists, nurses and educators. Muir Wood is in-network with most commercial insurers and is not in-network with Medi-Cal/Medicaid.

Ventura County Behavioral Health in Ventura is a provider of outpatient mental health services.

Washington County Mental Health Services operates a program site on Heaton Street in Montpelier, Vermont. The non-profit designated agency provides residential and outpatient support for substance use and co-occurring mental health conditions, alongside intensive adult mental health services and psychiatric and nursing care. Crisis response is available 24 hours a day across the agency's Washington County programs.

St. James Infirmary is an affordable, peer-based occupational health and safety clinic for sex workers of all genders. The facility accepts clients on opioid medication.

Center for Discovery Fairfield provides residential eating disorder treatment for women and non-binary adults 18 and older in Fairfield, Connecticut. The program offers evidence-based care and is an in-network provider for major health insurance plans, including Aetna, Cigna, Humana, and Optum. It accepts private insurance.

Community Mental Health Affiliates (CMHA) is a private non-profit behavioural health provider headquartered in New Britain, Connecticut, with eight locations across central Connecticut and in Waterbury, and was the state's first fully Joint Commission accredited Behavioral Health Home as well as a SAMHSA-certified Community Behavioral Health Clinic. Its residential continuum includes a 12-bed supervised programme staffed by an Intensive Assertive Community Treatment team, the 16-bed Parkview 5 - which CMHA describes as one of only two programmes at that level of care in the state, with an average stay of about 18 months and usually a waiting list - and a 19-bed Transitional Living Center that steps clients down from Parkview 5 with nursing cover and two respite beds. Horizon Health Services – Boulevard Recovery Center provides intensive outpatient (IOP) care for substance use disorders in Tonawanda, NY. The program serves adult men and women, young adults, seniors, with services that include cognitive behavioral therapy (CBT), trauma-related counseling, relapse prevention and medication-assisted treatment (MAT). Accepted payment options include private insurance, Medicaid, Medicare, military insurance.

Carolina Center for Behavioral Health in Greer is a provider of mental health care services.

Hector Reyes House is a residential substance use treatment programme for 25 Latino men in Worcester, Massachusetts, run by the Latin American Health Alliance to address the health inequities the alliance says Latinx residents of greater Worcester face. The house combines in-house medical treatment with intensive cognitive behavioural therapy in phases: an opening stage focused on establishing a daily routine and beginning intensive treatment, then a transitional phase in the second and third months when residents leave the house for outside activities and work on a balanced lifestyle as a bridge to living independently. Medicaid and Medicare are both recorded as accepted here, alongside private insurance and state-financed coverage.

ElevaCare is a community behavioral health center serving Nobles, Jackson and Cottonwood counties in southwest Minnesota. Grayson Youth and Family Services is the Mount Rogers Community Services Board location serving Grayson County, offering counseling and intensive outpatient care for youth and families in Independence, Virginia. The program addresses mental health and substance use concerns and offers same-day access intakes on weekdays. It accepts Medicaid, Medicare, private insurance, and military insurance.
