Center for Discovery in Fairfield, Connecticut (Wellington campus) provides residential eating disorder treatment. The program accepts commercial insurance and TRICARE; Medicare and Medicaid are not accepted.

Avanzando Enhanced Residential Rehabilitation is a Gándara Center program in Ludlow, Massachusetts, treating women 18 and older with co-occurring mental illness and substance use disorders. The 20-person program provides mental health and substance use treatment from a single team, including onsite medication-assisted treatment, plus postpartum care during and after pregnancy. It accepts Medicare, Medicaid, private insurance, state funding and military coverage.

Metrocare Services provides substance use, mental health, and developmental disability care at multiple locations across Dallas County, Texas. With more than 50 years serving North Texas, Metrocare's person-centered and trauma-informed programs include accessible pharmacies, veterans' services, and supportive housing in addition to clinical behavioral health treatment.

New Creation IOP is an intensive outpatient program in Ontario, California, serving residents of San Bernardino and Los Angeles counties. It offers a stepped continuum of partial hospitalization (PHP), intensive outpatient (IOP) at about 10–12 hours per week, and outpatient care, alongside a sober-living option and evening hours. The program accepts Medicare and private insurance.

Highlands Community Services is a Community Services Board providing outpatient and intensive outpatient treatment for mental health and substance use disorders at its Abingdon campus. The agency has served Washington County and the city of Bristol, Virginia for more than fifty years and operates a 24/7 crisis line. It accepts Medicaid, Medicare, private insurance, and military insurance.

Metrocare Services is Dallas County's public provider of mental health, developmental disability and substance use services, with a clinic at 9708 Skillman Street in Dallas, Texas. It offers intensive outpatient and outpatient treatment, medication-assisted treatment for adults and teens, detoxification, and veterans services, with a 24/7 mental health crisis hotline at (214) 743-1215. Metrocare accepts Medicare, military insurance and most private insurance.

Newport Integrated Behavioral Health Care provides residential, intensive outpatient and outpatient substance use treatment in Decatur, Georgia. The program serves adults and young adults, including veterans and people with co-occurring mental health conditions, and uses cognitive behavioral therapy, motivational interviewing and trauma-focused counseling. Buprenorphine is available for opioid use disorder, and Medicaid and Medicare are accepted.

Apalachee Center provides residential substance use treatment at its Quincy, Florida location, part of a network of behavioral health facilities serving the Big Bend region. The center offers inpatient and outpatient services, with programs for adults and adolescents. Apalachee Center is a community-based provider serving Gadsden County and surrounding areas. It accepts Medicaid, Medicare, and private insurance.

New York Center for Living in New York is an outpatient rehab and intensive outpatient program (IOP) for adults and teenager. The facility accepts clients on opioid medication.

Valley Vista provides medical detoxification and residential inpatient addiction treatment at its campus on Upper Plain in Bradford, Vermont, with separate programs for men and women. Treatment addresses substance use alongside co-occurring conditions including anxiety, bipolar disorder, depression, PTSD and trauma, and the program also serves veterans. Low-intensity inpatient care and aftercare planning support the step down from residential treatment.

Family Guidance Center provides outpatient and intensive outpatient behavioral health care from its Felix Street site in Saint Joseph, using a team approach across programs for substance use disorders, adult mental health and children and youth services. The agency runs offices across nine northwest Missouri counties and its staff respond to crisis situations throughout that nine-county area 24 hours a day, seven days a week, and medication-assisted treatment is also provided. Dakota Boys and Girls Ranch provides residential treatment for children aged 5 to 17 who need psychiatric care, therapy and support to heal from trauma, and has been working with North Dakota families for more than 70 years. Its Bismarck campus pairs treatment with specialised accredited education for young people who need extra support at school, and the Ranch describes its approach as combining spirituality and science, with trauma-informed care delivered by licensed therapists, psychologists, nurses and educators. Programs and schools are licensed by the state of North Dakota and nationally accredited; Medicare and private insurance are accepted.

Washington County Mental Health Services runs a program on Kynoch Avenue in Barre, Vermont providing residential and partial hospitalization level care. The non-profit designated agency supports adults with substance use and co-occurring mental health conditions through intensive day programming and residential support. It forms part of the agency's wider network of Washington County services.

Mount Juneau Counseling and Recovery, run by Gastineau Human Services Corporation on Aisek Street in Juneau, operates a 32-bed low-intensity residential substance use treatment program at ASAM level 3.1 together with outpatient individual counseling and groups, so clients can move from residential care through outpatient support. The organisation also runs the Glacier Manor Community Residential Center, a correctional rehabilitation program for up to 40 people, and its clinical team includes master's-level counselors and a residential program manager. Medicare, private insurance and self-pay are accepted.

ChangePoint Integrated Health has provided behavioral health services in Navajo County, Arizona since 1966. Its Winslow clinic offers outpatient counseling and psychiatric services for children and adults, including psychiatric evaluation and medication monitoring. ChangePoint accepts Medicare and Medicaid, and offers a sliding fee schedule without turning anyone away for inability to pay.
