Located in Galax, Virginia, the Life Center of Galax is a private drug & alcohol rehab center for the treatment of alcoholism, drug addiction and dual diagnosis problems. The Life Center opened in 1973 and was the first rehabilitation facility in Virginia specifically licensed and accredited to exclusively treat chemical dependency. Much of the treatment is based on the 12-step principles of Alcoholics Anonymous and Narcotics Anonymous.

Its work here is built around families rather than individuals alone: Parent-Child Interaction Therapy, family support services and parent aid sit alongside outpatient counseling, and treatment foster care places children who cannot remain at home. Clarvida of Virginia has provided trauma-informed, evidence-based care to children, adolescents, adults and families since 1992, and also offers autism services. The program takes Medicare alongside private and military insurance and state funding.

Alleghany Highlands Community Services has been Covington's community services board since 1983 — one of forty established under the Code of Virginia — providing outpatient and intensive outpatient treatment for substance use, mental health and co-occurring disorders to adults and children alike. Substance-using pregnant women and women with dependent children are seen within 48 hours of asking, and interim counseling is provided to priority populations while they wait for a residential, detox or intensive outpatient place to open. Information about services is published in English, Spanish, Korean and Vietnamese. State funding, Medicaid, Medicare and private or military insurance are all accepted.

Fernwood Counseling Center is the Mount Rogers Community Services Board's site for Carroll and Grayson counties and the city of Galax, offering counseling, psychiatry and intensive outpatient care for mental health and substance use disorders, with same-day access intakes every weekday — so a person who decides to seek help on a Tuesday morning does not wait weeks for an assessment. Mount Rogers is a not-for-profit governed by a fourteen-member board and has served southwest Virginia since 1972. Uninsured or underinsured clients may be given a capped fee under its sliding scale, or can apply for financial assistance; Medicaid, Medicare, private and military insurance and state funding are all accepted.

True North Health Clinic (Intercept True North) provides addiction treatment and psychiatric care in Roanoke, Virginia, with additional sites in Glen Allen and Fredericksburg. Services include intensive outpatient and partial hospitalization programs, residential treatment at ASAM Level 3.1, medication-assisted recovery and outpatient detoxification, plus statewide virtual services. Mount Rogers has served southwest Virginia since 1972 as a not-for-profit governed by a fourteen-member board, covering Bland, Carroll, Grayson, Smyth and Wythe counties. Frontier Health's Wise County Behavioral Health office in Norton, Virginia provides outpatient and intensive outpatient mental health and substance use treatment, including medication-assisted treatment. Frontier Health is a private, not-for-profit 501(c)(3) organization that has served the region since 1957 through more than 60 facilities across 12 counties in Northeast Tennessee and Southwest Virginia. The office accepts Medicaid, Medicare, private insurance, and TRICARE.

LewisGale's Salem site provides outpatient and intensive outpatient substance use treatment with withdrawal management, as part of the LewisGale hospital network in Virginia's Roanoke Valley. The program takes Medicaid and Medicare as well as private insurance, military coverage and state funding.

Frontier Health has provided behavioral health care across northeast Tennessee and southwest Virginia since 1957, and its Community Connections clinic on Champions Avenue in Big Stone Gap offers outpatient mental health and substance use treatment — individual and group psychotherapy, psychiatric services and crisis support. Addiction and co-occurring mental health conditions are treated together rather than in sequence, which is the organization's stated position on what makes recovery hold. Around the clinical work sits the rest of Frontier Health: vocational recovery services using the Individual Placement and Support model, which puts people into competitive employment with ongoing support rather than pre-employment training; developmental disability services; and sponsored residential home services. Sobrius operates a drug and alcohol addiction treatment center on Cliffview Road in Galax, Virginia, one of its three Virginia locations.
