Horizon Health and Wellness is a nonprofit integrated health care provider serving Arizona for over 40 years. This location provides intensive outpatient (IOP) and outpatient treatment for substance use and addiction, integrated with primary care and mental health counseling.

Joint Commission accredited and a Certified Primary Care Medical Home, Horizon accepts Medicare and private insurance. Its whole-person approach addresses both behavioral health and physical wellness under one roof.
