The organization was founded more than forty years ago in a barn in rural northeast Missouri and has grown into one of the largest community-based healthcare providers in the Midwest, working with approaches including trauma-focused cognitive behavioral therapy. Medicaid and Medicare are accepted, along with private and military insurance and a sliding fee scale.

Mental Health Centers of Western Illinois (MHCWI) is a CARF-accredited community behavioral health provider serving Brown, Hancock and Pike counties in western Illinois. Its Mt. Sterling campus at 700 SE Cross Street offers adult and child/adolescent mental health services, substance abuse treatment and residential programs, with a 24-hour after-hours crisis line. MHCWI is a nonprofit organization.

Alpharus Health Jacksonville provides residential addiction treatment with medically supervised withdrawal management (detox), alongside intensive outpatient and outpatient programs for substance use and co-occurring disorders. Part of a leading national nonprofit treatment provider, the center accepts Medicare and most major private insurance plans, including Aetna, Cigna, Humana, and United Healthcare. The provider announced its change of name from Gateway Foundation in August 2026.
